CUBE Collective LLC is not a healthcare provider, EHR vendor, or medical device manufacturer. CUBE OneCare is a care engagement, coordination, and benefits analytics platform. The BAA template becomes binding only once signed by both parties, and CUBE applies the corresponding HIPAA safeguards when PHI is processed on behalf of a Covered Entity. CUBE has signed a BAA with Stedi (clearinghouse); BAAs with the rest of our providers, including hosting, are pending and will be published in the CUBE control panel when available, and until they are in place OneCare does not process real PHI.
Obligations to implement administrative, physical, and technical safeguards for Protected Health Information
Permitted uses limited to performing contracted services; no unauthorized disclosures
Notification of any breach of unsecured PHI within 24 hours of discovery
Requirement for subcontractors to agree to equivalent BAA terms
